PREVIOUS RECIPIENTS ARE NOT ELIGIBLE TO APPLY.

The following four (4) required documents must be typed and uploaded via the application. HANDWRITTEN APPLICATIONS WILL NOT BE CONSIDERED.
1) The completed application of students from Illinois medical schools. (completed via the link)
2) An official statement of need from your medical school’s Financial Aid Office.
3) A NON-PDF application essay or personal statement of 500 words or less.
APPLICATIONS EXCEEDING 500 WORDS WILL NOT BE CONSIDERED.
4) Resumé or Curriculum Vitae (CV). Describe your proven leadership skills, your
extracurricular commitment to social justice and demonstrated promotion of
health in the African-American community.
The deadline for submission of all applications/materials is 11:59 PM CST March 8, 2024
The scholarship recipients will be notified Wednesday, March 22, 2024.

Underrepresented in medicine (URM) means those racial and ethnic populations that are underrepresented in themedical profession relative to their numbers in the general population [Adopted by the American Association of
Medical Colleges (AAMC) Executive Committee, March 19, 2004] URM can be defined as African American/Black, Latina/Latino, Alaskan/Hawaiian Native, Hispanic American, and Native American.
